coronary microvascular dysfunction

 24.2.9: web.health/heart/
coronary microvascular dysfunction:

summary:

. if you feel like a heart attack is happening

but they can't find an obstructed or clogged artery,

that is known as MINOCA:

myocardial infarction [heart cell death due to blood loss]

with non-obstructive coronary arteries

which can be caused by artery spasms, ripped artery,

or a new condition that is known by many names:

coronary microvascular dysfunction

Coronary microvascular disease

Microvascular endothelial dysfunction

Cardiac syndrome X

Nonobstructive coronary heart disease.

The Association of Medical Illustrators is

excited to announce the launch of

the first round of illustrations through the

AMI Diversity Fellowship,

made possible by a grant through Johnson & Johnson’s (J&J)

Our Race to Health Equity (ORTHE) initiative.

In recognition, we will be highlighting the work of

each of our Diversity Fellows.

eg, the first illustration highlights

myocardial infarction with non-obstructive coronary arteries.

[!] image: myocardial infarction with non-obstructive coronary arteries

(MINOCA), can be caused by artery spasms

or coronary microvascular dysfunction.

interesting that mentions

"coronary microvascular dysfunction";

it is a new diagnosis with many names...

Small vessel disease

https://www.mayoclinic.org/diseases-conditions/small-vessel-disease/symptoms-causes/syc-20352117

Small vessel disease may also be called:

Coronary microvascular disease

Microvascular endothelial dysfunction

Small vessel disease is treatable but may be difficult to detect.

The condition is typically diagnosed after finding

little or no narrowing in the main arteries of the heart

despite the presence of symptoms that suggest heart disease.

Coronary Microvascular Disease (MVD)

https://www.heart.org/en/health-topics/heart-attack/angina-chest-pain/coronary-microvascular-disease-mvd

In coronary MVD, the heart's coronary artery blood vessels

don't have plaque, but damage to the inner walls of the blood vessels

can lead to spasms

or abnormalities in smaller arteries.

Other names for coronary microvascular disease:

Cardiac syndrome X

Nonobstructive coronary heart disease

Treatment

Medications to relax blood vessels including

beta blockers, calcium channel blockers and nitroglycerin.

What is MINOCA? A Type of Heart Attack Mostly Affecting Women

. myocardial infarction with non-obstructive coronary arteries

(MINOCA), is garnering increased attention.

Janet Wei, MD, and Noel Bairey Merz, MD:

MINOCA is a heart attack caused by

poor blood flow to the heart,

despite no evidence of major blockage in the arteries,

eg, coronary artery spasms, and

spontaneous coronary artery dissection.

A troubling aspect of MINOCA is that

often when we diagnose a heart attack,

we rely on a single imaging method

—invasive coronary angiogram—

to tell us if there’s a big blockage in the coronary arteries.

With MINOCA, the angiogram can look normal.

MINOCA patients are more likely to be female and younger,

and they’re also more likely to be Black or Hispanic/Latino.

the unexpected hazards of being a statistic

 2022.6.22: pol/healthcare/
the unexpected hazards of being a statistic:

. there was a problem with exploding air bags that killed people

but they didn't have them disarmed until replacement

because statistically, if they did disarm the air bag

there would be more people dying from accidents

than there would be deaths from air bag explosions

because seat belts alone are far deadlier than

seat belts with air bags.

. but those statistics just show the cost to society;

what about the cost to my family

when they know I may never have died from an accident

but I did die from an air bag explosion?

suggestions for a globalized pandemic preparedness and response

 2022.4.12: co.who.int/pol/healthcare/
suggestions for a globalized pandemic preparedness and response:

the intro from WHO:

https://inb.who.int/home/written-submissions

the form to give your comment:

https://extranet.who.int/dataformv3/index.php/491121?lang=en

the question:

“What substantive elements do you think should be included

in a new international instrument

on pandemic preparedness and response?”

my comment:

. one thing we should do to prepare

is teaching people about who survived the pandemic;

it was people with a diet that promotes

low inflammation, low cortisol, and low insulin,

for enhanced insulin sensitivity.

. and for elders it was high levels of vitamin D, vitamin A,

with vitamin K2 to prevent bone loss from vitamin D,

melatonin, and zinc.

. we don't want a global effort of

forcing everyone to take risky vaccines,

when we could work to isolate the vulnerable,

while we rapidly build natural immunity,

and get the lock-downs over with sooner!

to optimize a boy's IQ, avoid fluoridated water during pregnancy

2022.3.29: news.health/diet/fluoride/
to optimize a boy's IQ, avoid fluoridated water during pregnancy

. a lower IQ is what makes lead so dangerous,

increasing risky behaviors.

. if your region's water has high or added fluoride,

drinking that water while pregnant with a boy

will lower that boy's IQ.

tea is another major source of fluoride.

. this is not a call to end water fluoridation,

but to drink pure water during pregnancy.

Cares Act 2020 required removing guaifenesin from asthma meds

 21.7.12: web.health/diet/

Cares Act 2020 required removing guaifenesin from asthma meds:

. why was primatene recalled?

a competitor claimed that the Cares Act 2020

required removing guaifenesin;

possibly due to it being known that

guaifenesin may cause birth defects.

but that was known at least as far back as 2003.

aapsonline.org worries more "democracy" is a path to socialized medicine

2021.3.2: news.pol/healthcare/

aapsonline.org worries more "democracy" is a path to socialized medicine:

summary:

H.R. 1: For the People Act 3 of 2021:

https://www.congress.gov/117/bills/hr1/BILLS-117hr1ih.pdf

"To expand Americans’ access to the ballot box,

reduce the influence of big money in politics,

strengthen ethics rules for public servants,

and implement other anti-corruption measures

for the purpose of fortifying our democracy, ... ."

. we actually need this bill to protect democracy

by making it easier to vote;

but it is bound to help the Democrat.party

which is known for increasing socialized medicine

which is bad if it limits the ability of doctors

to decide what they consider good patient care

instead of letting politicians decide your care.

. this is why Republicans voted for abortion rights:

https://www.worldatlas.com/articles/roe-versus-wade-decided-by-a-majority-republican-nominated-court.html

they actually voted to keep politicians from

taking choice from our doctors.

. I hope when this bill allows more Democrat politicians

that they help expand our access to healthcare

without tying the hands of doctors.

. if the usa has the most deaths from covid-19

https://en.wikipedia.org/wiki/COVID-19_pandemic_death_rates_by_country#/media/File:COVID-19_Outbreak_World_Map_Total_Deaths_per_Capita.svg

it was because we didn't give doctors more power:

good doctors wanted to treat early with older

unpatented drugs such as Ivermectin

and zinc with HCQ; and vitamin D;

[Am J Med. 2021 Jan]

https://pubmed.ncbi.nlm.nih.gov/32771461/

whereas, the socialized medicine in the usa

told doctors not to treat until hospitalization was needed,

and to use only new patented drugs and ventilators.

safe harbor provisions of the Anti-Kickback Statute

2021.2.1: pol/healthcare/

safe harbor provisions of the Anti-Kickback Statute:

to the HHS Office of Inspector General (OIG):

https://www.regulations.gov/comment?D=HHSIG_FRDOC_0001-0440

Thank you for the opportunity to provide comments on

abuse of safe harbors to the anti-kickback statute (AKS).

. this letter is a revision of the aapsonline response.

https://aapsonline.org/alert-and-special-legislative-update-pharmacy-benefits-managers/

Physicians Against Drug Shortages estimates that

http://www.physiciansagainstdrugshortages.com/

safe harbor facilitated kickbacks increase

"annual supplies costs by at least 30%,

or upwards of $100 billion

... and the prices of drugs ...

by at least $100 billion annually as well."

I urge HHS to move forward with the Part D rebate rule

instead of delaying it further.

. that regulation would replace protections for

drug rebates in Medicare Part D

with protections for discounts

provided directly to consumers.

. the Biden administration agreed to delay this rule

in response to suit challenging it brought by

the Pharmaceutical Care Management Association.

https://www.fiercehealthcare.com/payer/a-win-for-pbms-biden-administration-delays-rebate-rule

It is past time that discounts on drugs

are passed along directly to patients

instead of being kicked back to middlemen.

In addition, the Office of Inspector General (OIG)

must not sidestep its responsibility to investigate and end

abuse of current AKS safe harbors,

for example 42 CFR § 1001.952(j) and related regulations.

This safe harbor for Group Purchasing Organizations (GPOs)

has unleashed an epidemic of kickbacks

in the medical supply chain

which has spread into pharmaceuticals

through similar exploitation by

Pharmacy Benefits Managers (PBMs).

HHS should further work to stop both

GPO and PBM misuse of safe harbors

by encouraging Congress to repeal 42 U.S.C. § 1320a-7b(b)(3)(C),

a key statute that shelters harmful schemes.

Shortages, unaffordability, and improper steering of patients

to the most profitable product, instead

of the one that best meets patients' needs,

are driven in part by this AKS safe harbor.

For example,

sole-source contracts, facilitated by kickbacks,

are an anti-competitive feature of

many GPO arrangements with hospitals and manufacturers,

which increase the risk of shortages and push prices higher.

because a lot of bleeding was involved,

however, there was a new popular fad at the time,

that was causing the same sort of bleeding
that is caused from scurvy or low vit'C.

non-alcoholic fatty liver disease is very common #fructose #roundup

or from getting too many antibiotics

bad germs or leaky gut and being obese,

[J Hepatol. 2008]

Mary E. Rinella, MD:

Nonalcoholic Fatty Liver Disease
A Systematic Review

Nonalcoholic fatty liver disease
affects approximately 30% of the US population.
p2266:
high visceral adipose tissue (being obese)
increases pro-inflammatory mediators
and excessive free fatty acids (lipotoxemia)
and insulin resistance;
all that affects the liver:
alterations of glucose and lipid metabolism.
. all that altered metabolism leads to
fatty liver or hepatic steatosis.

2nd stage is steatohepatitis
caused by lipotoxicity from increased free fatty acids
and by inflammation from
obesity releasing pro-inflammatory mediators,
and by intestinal dysbiosis generating portal endotoxin.
. pro-inflammatory mediators and
(endoplasmic reticulum sress
from dysregulated unfolded protein response)
leads to pro-inflammatory signalling
that along with (the vicious circle of
oxidative stress and mitochondrial dysfunction)
causes activation of hepatic stellate cells
and collagen deposition;
that leads to fibrosis
which is a precursor to cirrhosis.

. one symptom of insulin resistance is
acanthosis nigricans
[velvety darkening of the skin.
It is usually found in body folds.]
